Our staff is highly trained on the National best standards of practice to achieve optimal patient outcomes. Our goal is to promote and improve patient understanding of disease management.
Primary care physicians serve as the trusted and dedicated healthcare providers for patients, playing a vital role in closely monitoring their health, prescribing necessary medications, and collaborating with other members of the care team. With their expertise, they assume responsibility for supervising and approving the patient’s personalized Plan of Care.
Collaborating seamlessly with primary physicians, hospice medical directors work hand-in-hand to oversee and endorse the patient’s Care Plan. In some cases, they may even step in as the primary physician, if required. These medical directors provide the initial terminal prognosis, conducting face-to-face visits to ensure the patient’s comfort and continuously assessing their eligibility for hospice care.
Registered nurses take on the role of the patient’s dedicated case manager, equipped with the skills to assess symptoms and medical needs. They excel in managing pain and medication, striving to maintain optimal comfort levels. Responsible for the patient’s overall physical well-being, these nurses also coordinate the patient’s care with the interdisciplinary team, ensuring seamless collaboration.
Hospice aides, who are certified nursing assistants, provide invaluable assistance with activities of daily living, including bathing, toileting, transferring, and light housekeeping. Working closely with nurse case managers, they contribute to the patient’s comfort and peace, delivering care that prioritizes their well-being.
